Hello, mysterious traveler... I am delighted to invite you to participate in October's GWYG. For these curious apothecary-inspired designs, here is how it works:
- There are 13 spots available and 15 total designs in the pool
- Designs measure approximately 3" x 3"
- Your apothecary treasure can be tattooed in Black & Gray for $200 or in color for $250 (color customizations are welcome).
- Additional rerolls are an additional $20 fee.
- Arm and leg placements are preferred, but I am open to other placements if you have an idea that will suit the design well!
- To book the GWYG, you simply need to submit this form. This GWYG is available to book between 10/1/25 through 11/1/25. Once I process your submission, we can pick a date and time, then the shop will send you an invoice for a $50 deposit that goes to the final cost of the tattoo.
- If you fail to complete the deposit within 3 days after it is sent to you, your spot may be forfeited and offered to the next person in line.

Deposits and Setting an Appointment (please check each box to confirm you understand the following)

I am over the age of 18.I am able to pay a deposit fee to confirm our agree upon appointment time and date within 48 hours of setting the tattoo appointment. The GWYG appointment deposit is $50. Failing to pay the deposit fee will forfeit the reserved appointment date and time slot.I understand that a deposit invoice will be sent from Lucky Goose Tattoo to my email or cell phone number that is provided on this form after confirming an appointment date and time with the artist.I understand that appointments must be rescheduled prior to 48 hours before the scheduled time and date. Failure to properly notify the artist in that time frame or "no call/no showing" to the appointment may result in the forfeiture of the deposit.
