Consent and Acceptance
1. Having read the information sheet and understood the level of supervision to be provided, I agree to my child taking part in the program.
2. I understand that all reasonable care will be taken of my child during the adventure program and that he/she will be required to follow all directions and instructions given, and observe all rules and regulations governing the activity.
3. I certify that the information provided above is true and correct.
4. I give permission for any photos/videos taken during the program to be used by Wholesome Education Limited for promotional purposes.
5. Program fees are non-refundable and non-transferable. There will be no refund or make-up class for no-shows, absences, sick leave, cancellation due to inclement weather, or suspension/cancellation announced by the Education Bureau (EDB).
6. Wholesome Education Limited reserves the right to change instructors, time, venue, and/or content of the program. If the number of enrolments is insufficient to run the class, we reserve the right to cancel it at our sole discretion, as we deem appropriate.
