PARENTAL CONSENT FORM
The Humane Society of the Felicianas ("HSOTF") values your children and we want to ensure their safety while they are volunteering at the animal shelter. Youth volunteers must be 16 years of age or older to volunteer at the shelter, without parental supervision. Under the age of 16 are welcome to volunteer. However, they must be accompanied by a parent or guardian over the age of 18. Any volunteer under the age of 18 must have a Parental Consent Form, signed by the parent or legal guardian, on file.
I,Parent/Guardian Name*, the parent/guardian of Minor's Name* do hereby give permission for my minor child to volunteer at HSOTF. I understand that my child will be working with and around animals, and may be asked to do physical labor such as sweeping, mopping and cleaning. I understand that my child may be handling dogs and cats of all ages. I hereby knowingly, freely and voluntarily waive any right or cause of action of any kind whatsoever arising as a result of such activity from which, any liability may or could accrue, again HSOTF or its agents or employees, jointly or individually.
