VIDEOS ARE DUE 1 MONTH PRIOR TO THE TESTING DATE

Students testing for green belt rank or above must submit their video test as a YouTube link below. Make sure all information is accurate and only submit your link one time. Please note, video tests take up to 4 weeks to review. Results and confirmations will be emailed to the email address you provide below. Questions - email coach.v@kaizenkaratemd.com. Lastly, students must register and pay for the test PRIOR to submitting the video links below.

Video tests will not be accepted IF the student does not have the appropriate patches sewn onto their gi and placed correctly on the uniform. NO EXCEPTIONS. See here for details - www.kaizenfitnessusa.com/patches. Patches must be visible on video test that you are submitting.

Note - There is no video test from White to Yellow Belt. Please enter "NA" in the section below to proceed.

POLICIES

1. Instructors are NOT responsible for filming students during class time and / or submitting videos on the students behalf.  It is the responsibility of the parent to film and submit videos for grading.
   
   
2. I understand that my child is expected to film their test on their 1st attempt.  Students should film their video on their first try as if they were being graded live and in-person.
   
   
3. Students should be visible from head to toe during the entire video.  If a student is out of frame, the video will be sent back and the student will be asked to re-film their test.
   
   
4. I understand that videos are due 1 month prior to the actual testing date.  If videos are submitted after the deadline (see website for deadlines), then the student must wait until the next testing cycle.  NO EXCEPTIONS.  We hold tests every two months so the wait is relatively brief.
   
   
5. I understand that if the videos are "incomplete" that my test will automatically be pushed back to the next testing date.  Eg. If a student sends in technique and kata videos and does not send in sparring videos, they will automatically be pushed back to the next testing cycle.
   
   
6. I understand that if my child does not pass the video test (or receives directions to film the videos again), then we must automatically wait for the next testing cycle.  This will allow time to fix the mistakes in the test.
   
   
7. I understand that if my child needs to submit videos again, they must work directly with either Coach V or Coach David to fix all issues in the test (not with their primary instructor).
   
   
8. I understand the ultimate goal is to get to black belt.  In order to do so, we need to make sure that each student's skill matches their belt and their belt matches their skill.  Moving up in belt rank is not a race.  It is important to take time at each rank to properly learn all material.  
   
   
9. I understand that all video tests must be submitted on this form.  At no time should students submit video tests directly to their instructor.  Instructors are also aware that students should at no time submit videos to them for "review" prior to the actual video test submission.  Any violation of this policy must be reported directly to Kaizen Karate's Head Instructor, Coach V.
