2025 Spirit of Excellence Awards Nominee Form

Hosted by the Yarmouth Chamber of Commerce and the Rotary Club of Yarmouth, the Spirit of Excellence award program recognizes and celebrates non-managerial employees from businesses and non-profit organizations in the Yarmouth area.

Participating companies and organizations are invited to select an employee (no more than two) to be recognized based on the following criteria: A member of your staff in a non-managerial position; Demonstrates outstanding commitment to the business; Serves as a role model to other employees; Makes a real difference to the company.

We are planning our surprise visits for the week of October 20th, please tell us the Best Days/Times for us to stop in with award recognition. We will call to confirm a final date and time one week in advance.
Award Ceremony Event:

We are planning an in-person celebration for Thursday November 6th from 5-8pm at Patriot Insurance. Supervisors should plan to accompany their candidate to the celebration. The cost of the event is $25 per person, and nominees attend as our guests. Feel free to invite the recipient’s family and colleagues for the surprise visit or to the Award Ceremony dinner. If you would like to reserve a table, please lets us know in advance.
