Release of Liability and Acknowledgement

Activity Description:

This Release of Liability Form pertains to all basketball training programs, clinics, tournaments, and related activities ("Activities") conducted by GFB Academy and or GFB Elite Basketball Academy.

Acknowledgement of Risks

Participants and their guardians acknowledge that basketball and related Activities involve inherent risks of injury, illness (including communicable diseases such as MRSA, influenza, and COVID-19), and in some cases, permanent disability or death. Despite strict safety protocols, equipment, and personal discipline, the risk of serious injury and illness exists.

Voluntary Participation

By signing this form, participants and their guardians confirm their understanding of these risks and voluntarily assume all known and unknown risks associated with these Activities.

Release of Liability

By participating in GFB Elite Basketball, participants and their legal guardians hereby release and hold harmless GFB Academy, GFB Elite Basketball and their respective directors, officers, employees, volunteers, associates, and agents from any and all claims, liabilities, or demands arising from injury, illness, or damage sustained during participation, including those resulting from the ordinary negligence of the parties released.

Indemnification

Participants and their legal guardians agree to indemnify and hold harmless GFB Academy, GFB Elite Basketball, and all affiliated personnel and representatives from any and all liabilities, claims, or expenses arising out of or related to the participant’s involvement in program activities.

Media Release

By participating in GFB Academy and GFB Elite Basketball activities, participants and their guardians grant GFB Academy and GFB Elite Basketball permission to record, capture, and use video, audio, and images of the participant for promotional, educational, and marketing purposes. This includes, but is not limited to, use on GFB Academy's and GFB Elite Basketball's website, social media platforms, brochures, advertisements, and other promotional materials. Participants and their guardians waive any rights to review, inspect, or approve the final product and understand that they will not receive compensation for the use of such media.

Agreement

I have read this Release of Liability and Acknowledgement Form, fully understand its terms, and sign it freely and voluntarily without any inducement. I understand that I am giving up substantial legal rights by signing this form.
