Tattoo Submission Form
Please read before filling out form:
- There is a $100 non-refundable deposit for every appointment scheduled. This amount goes towards your tattoo. Deposits are transferrable if rescheduling is needed but keep in mind guest spot availability is very limited.
- Your appointment is not officially booked until I have recieved your deposit.
- Form of payments accepted are card payments through square or zelle and/or cash at the time of your appointment. (Deposits must be paid digitally in advance)
