Fill out the form below to apply to be a Vendor at the Healthy Living Expo at McGee Park in Farmington, NM. Booth spaces are free but an application is required. Please complete this entire questionnaire with as much detail as possible. THANK YOU!

VENDOR NOTE:

Applications will be accepted until spaces are filled. Preference will be given to applications received first, space is limited. Applications will be evaluated on their connection to healthy living in San Juan County as well as balancing non-profit and for-profit participation. Completion of an application does NOT guarantee an acceptance. We reserve the right to refuse any application. In the event that a vendor needs to cancel, please notify sjhc@sjcpartnership.org as soon as possible.

ANY vendor found in violation of this agreement may be asked to leave and be prevented from participating in future events.
