  • By submitting this form I agree to the following terms and conditions:

      • I agree that the dog is being fostered by myself and will not be sold, adopted, or given to another party.

      • I agree that the dog will not be allowed outdoors without supervision.

      • I agree when taking my foster dog outdoors, s/he will be on a secure harness and at all times on leash, and wearing proper ID.

      • I agree that a foster dog is to be a companion animal, not a guard dog. The dog will live inside my home.

      • I agree that the foster dog must be de-sexed by 6 months of age. If the dog is not spayed/neutered prior to fostering, I will contact SDC to arrange for my foster dog to be de-sexed at 6 month of age.

      • I agree to care for the dog in a humane manner. This includes supplying adequate food, water, shelter, attention, and medical care.

      • I agree that if at any point I cannot keep the dog, I will return him/her to SDC without requesting any compensation.

      • I understand and agree that SDC makes no guarantees about the dog's temperament and is not responsible for future damages or injuries caused by the dog.

      • I give SDC permission to call or visit my home at any reasonable time to assure that the dog is being properly treated and cared for. 

      • SDC will have the right to remove a dog if they feel the environment is considered to be unsafe, unloving or unhealthy for the dog. The foster shall be held responsible for the costs incurred due to the necessitated removal of the dog. This shall include any veterinary costs related to the reason/s for the removal of this dog.

      • If the dog is lost or dies, the foster is obliged to inform SDC immediately.

      • I agree to hand over the dog to SDC once my foster period is over. (failure/refusal to hand over a dog to the rescuer will lead to legal action being taken)

      • I confirm that the foster dog will remain under the care of SDC and I will not try to keep the dog without going through the proper adoption process.

      • I agree to keep SDC informed of my current home address and phone number.

      • I agree that all statements I have made on this form are true. If it is found that any statements I have made on this form are not true SDC can remove the dog from my care.
