NOTE: The District's ability to match you with a Snow Angel volunteer depends on the amount of volunteers that enroll in the program. Please help us out by encouraging friends, family, and neighbors to volunteer to be a Snow Angel!

I agree that I have voluntarily requested help/service from this Snow Angel Program. I understand the program is Bartlett Fire Protection District-led and that a local neighbor/volunteer will come to my home (if safe to do so) to shovel or salt after a major snow event. On behalf of myself and any of my personal representatives, heirs, and next of kin, I hereby covenant not to sue and to hold harmless, release, and indemnify the neighbor helping me (the volunteer Snow Angel) and/or this Bartlett Fire Protection District-organized and Bartlett Fire Protection District-led initiative from any and all liability, claims, demands, actions and causes of action whatsoever, brought by any party against any party, arising out of my participation (whether caused by negligence or otherwise) in this "Snow Angel" program. I recognize that my participation in this volunteer "Snow Angel" program shall not include any interactions or communications with my match / Snow Angel except to the extent necessary to perform requested snow removals. I further recognize that no entry inside the residence(s) is permitted as a part of this program. I acknowledge that participation is strictly voluntary and will not yield any compensation in any form. I/we warrant and represent that I/we are the owner(s) of the property described above and hereby authorize the snow removal volunteer work to be performed at my/our property.
By clicking SUBMIT below I/we hereby waive and release any cause of action / claim / demand due to any injury and/or damage which may be sustained as a part of this completely voluntary program.
