    This form must be completed and submitted prior to your scheduled appointment. Failure to complete this form may result in your appointment being cancelled. By booking an appointment with Shields Auto Glass LLC, you acknowledge and agree to the following:
  • Appointments, Quotes & Service Area
    All estimates are valid for 30 days from the date issued and are subject to change if part prices or availability change after that time.
    All Shields Auto Glass technicians are AGSC-certified and have passed background checks.
    Appointments must be canceled or rescheduled at least 24 hours in advance. Missed appointments or late changes may result in a $50 fee at the discretion of Shields Auto Glass. Please allow an arrival window of 15–30 minutes for your technician to arrive for your scheduled appointment.
    A travel fee may apply for service outside our standard service radius of Kuna, Meridian, Star, Nampa, Caldwell, Boise, and Eagle. Please call to confirm pricing for your location.
    Severe weather or material shortages may affect your scheduled appointment. Mobile service may be delayed or rescheduled for rain, snow, high winds, or extreme temperatures. We will notify you as soon as possible and work quickly to reschedule.
    We reserve the right to refuse service to anyone at our discretion, for any reason, including safety concerns, location accessibility, or customer behavior.

    Vehicle Access & Customer Responsibilities
    The customer must provide a safe, flat work area with at least 5 feet of clearance on all sides of the vehicle.
    The vehicle must be unlocked and accessible at the scheduled time, and all personal items must be removed from the work area.
    If these conditions are not met, the appointment may be canceled or rescheduled at our discretion and may incur a service fee.
    The customer agrees to disclose any prior damage or repairs to the affected area before work begins.
    The customer understands that while every precaution is taken, older trim, moldings, clips, or hardware may break due to age or brittleness during removal, and Shields Auto Glass is not responsible for damage to surrounding parts or paint that occurs despite careful handling.

    For remove and reinstall (R&R) services, Shields Auto Glass is not responsible if the existing glass cracks, chips, or breaks during or after removal. Previously installed glass can contain hidden stress points or weaknesses that may cause it to fail despite careful handling. Replacement costs for broken glass during R&R are the responsibility of the customer.

    Payment Policy
    Full payment is due at the time of service unless prior arrangements are made.
    If the customer will not be present at the appointment, full payment must be made in advance.
    We accept cash, all major credit/debit cards, approved third-party billing, and verified insurance claims.
    A processing fee of up to 3% may be applied to card payments.
    All payments are final once processed — please review your invoice carefully before paying.
    Overpayments must be reported within 3 business days to be eligible for a refund.

    Workmanship Guarantee
    We stand behind every installation — all windshield replacements are backed by a lifetime warranty against leaks and installation defects.
    Our guarantee covers the quality of our work only and does not cover any damage occurring after installation.
    Warranty does not cover rock chips, road damage, or stress cracks caused by temperature changes after installation.

    Installation Safety & Aftercare
    After installation, the vehicle must remain parked for at least 1 hour (safe drive-away time) as advised by the technician.
    Any tape or clips must remain in place for 24 hours after installation.
    Do not take the vehicle through an automated or power car wash for 7 days after installation to allow the adhesive to fully cure.
    In rare cases, pre-damaged or cracked glass may shatter during removal. Shields Auto Glass is not liable for additional damage when attempting to safely remove pre-damaged glass.
    Shields Auto Glass is not responsible for pre-existing cracks, chips, rust, leaks, or molding/hardware issues not directly caused by our installation.

    Rock Chip Repair Information
    Rock chip repairs are performed to help prevent damage from spreading, but the chip may remain visible after the repair.
    There is no guarantee the repair will stop the crack from spreading during or after the repair.
    If the chip spreads during or after the repair, Shields Auto Glass is not responsible for additional damage or replacement costs.
    Chips or cracks larger than a quarter or near the edge of the windshield are less likely to be successful.
    Repairs are not covered by the lifetime warranty and are only recommended when the chip is not in the driver’s direct line of sight.

    Calibration & Diagnostics Disclaimer
    Shields Auto Glass does not perform ADAS calibrations.
    If a calibration is required after installation, it is the customer’s sole responsibility to schedule and pay for separately.
    Some vehicles may display warning lights or require sensor resets after service. Shields Auto Glass is not responsible for diagnostics, resets, or recalibrations after installation.
    Shields Auto Glass is not liable for safety issues or system malfunctions caused by an unperformed or incomplete calibration.

    Insurance Coverage Disclaimer
    We are not responsible for delays or claim denials from your insurance provider.
    Please confirm your coverage and deductible before scheduling service to avoid unexpected costs.
    Shields Auto Glass is not responsible for denied claims, unpaid balances, or incorrect deductible information provided by the customer or insurer.

    Glass Quality & Manufacturer Standards
    Minor optical distortions or manufacturer imperfections in new glass can occur and are considered normal within industry standards.
    Sudden temperature changes (hot/cold) can cause stress cracks, which are natural and not related to installation.

    Customer Property & Liability
    Shields Auto Glass is not responsible for personal items left inside the vehicle or for resetting radio, cameras, sensors, or ADAS features following service.
    Liability for any claims related to service will not exceed the cost of the work performed. Shields Auto Glass is not responsible for any indirect costs such as rental vehicles, lost wages, or loss of use.
