All surveys submitted will be reviewed by the Evidence Based Practice Committee. Submission deadlines are January 15, April 15, and September 15 at 11:59pm CST. Applicants will be notified of the committee's decision and provided feedback on their survey the following month. 

You may submit your survey at any time. For example, if you have your survey ready for submission in July, you can upload your survey in July for the September deadline.
