Consent for Aces Tennis Academy to use my child/ren's image, likeness, and/or voice (media)

1. I understand that my child/ren's media may be used publicly without further notification or compensation.

2. I understand that my child/ren's media may be edited, altered, or used in whole or in part without restriction, provided the use is consistent with the purposes outlined above.

3. I understand that my child/ren's media may be used indefinitely unless I withdraw consent in writing.

4. I acknowledge that Aces Tennis Academy will store this media securely in accordance with the Australian Privacy Act 1988 (Cth) and that I have the right to request access to it or have it removed at any time.

5. I understand that once media is published online, Aces Tennis Academy cannot control the distribution or use by third parties.

6. By consenting I understand that my/my child/ren's image, likeness and/or voice (media) may be used for the purposes of website content, social media organic and paid channels (e.g. Facebook, Instagram), printed publications (e.g. brochures, flyers), promotional videos, public presentations and events, media releases and news stories.

You have the right to withdraw your consent at any time by contacting Aces Tennis Academy. To withdraw consent please email social@acestennisacademy.com.au

Upon withdrawal of consent, no further use of your media will be made, though previous publications cannot be recalled.
