Applications must be filled out by an ADULT and the person who will be the guardian/owner of the puppy. Please note all information given in this questionnaire will be kept private between the person who applies and ourselves. It is not sold, distributed or shared. Some of these questions may seem personal, but they are all intended to be used to make sure that potential homes are suitable and to help match dogs to their lifetime homes.
