In the meantime, please continue care with your current family doctor or nurse practitioner, if you have one. It’s important to maintain your ongoing care until your registration with Noongwa Wellness is confirmed.

ConnectMyHealth
Signing up for ConnectMyHealth is a great way to prepare for your first visit with a new care provider. This free digital tool securely brings together your hospital health records from across the Ontario Health West Region into one place. Having your records easily available can help your new provider quickly understand your medical history and provide the best care.
If you are 16 or older, have a valid Ontario Health Card, and a unique email address, you can register for an account. Parents and Legal Guardians of youth aged 16–17, and Legal Representatives of adults 18+, can also register as Proxy Users to access records on the patient’s behalf. Support is available if you need help signing up.
MedsCheck
If you're taking three or more medications for chronic conditions, managing diabetes, or living in long-term care, consider contacting your pharmacy for a MedsCheck. This complimentary service offers a comprehensive medication review conducted by your pharmacist, ensuring the safe and effective use of your prescriptions.
