Program Purpose
The Community Innovation Center (“CIC”) is offering a volunteer-based Errands & Delivery Program to assist community residents with free grocery delivery, free notary services and other essential errands. This program is provided as a courtesy to support residents who may feel unsafe or unable to leave their homes.
Consent & Acknowledgment
By signing below, I acknowledge and agree that:
Voluntary Participation
I understand that CIC staff, volunteers, or contractors are not licensed delivery professionals but are assisting in good faith.
No Guarantees
CIC does not guarantee the availability of services, timeliness of delivery, or condition of goods purchased or delivered.
Assumption of Risk
I understand that, despite safety precautions, there are inherent risks associated with errands and deliveries, including but not limited to accidents, theft, property damage, injury, or delays.
I voluntarily assume all risks associated with my participation in this program.
Release of Liability
In consideration of receiving services, I hereby release and hold harmless the Community Innovation Center, its officers, employees, volunteers, and agents from any and all liability, claims, demands, or causes of action arising out of or related to participation in this program.
This includes, but is not limited to, any injury, illness, property damage, or loss sustained in connection with errands or deliveries.
No Medical or Legal Responsibility
CIC is not responsible for medical decisions, prescription errors, or verifying the appropriateness of requested items.
I am solely responsible for ensuring that requested items (e.g., groceries, prescriptions) are accurate and lawful.
Illinois Law Notice
This waiver is intended to be as broad and inclusive as permitted by the laws of the State of Illinois. If any portion of this waiver is held invalid, the remaining provisions shall continue in full legal force and effect.
