Emergency Information

Informed Consent and Acknowledgement

I, the Parent/Guardian, hereby freely and voluntarily, without duress, execute this Release under the following terms:

 1. Waiver and Release – I hereby release and forever discharge and hold harmless ArtHaus Foundation, Inc. and its successors and assigns from any and all liability, which may hereafter arise from child’s participation with ArtHaus Foundation, Inc. and/or any project, activity, or event sponsored, managed, arranged, or promoted by, or otherwise affiliated or associated with ArtHaus Foundation, Inc.. I understand and acknowledge that this Release discharges ArtHaus Foundation, Inc. from any liability or claim that I may have against ArtHaus Foundation, Inc., with respect to any bodily or other injury, illness, death, or property damage that may result from my child’s participation. I also understand that ArtHaus Foundation, Inc. does not assume any responsibility or obligation to provide financial assistance or other assistance, including, but not limited to, medical, health, or disability insurance, in the event of injury, illness, death, or property damage.

 2. Medical Treatment – Except as otherwise agreed to by ArtHaus Foundation, Inc. in writing, I hereby  release and forever discharge ArtHaus Foundation, Inc. from any and all liability claims, demands, and causes of action whatsoever that may arise on account of any first aid or other medical treatment rendered during my child’s participation with ArtHaus Foundation, Inc. and/or any project, activity, or event sponsored, managed, arranged, or promoted by, or otherwise affiliated or associated with ArtHaus Foundation, Inc.

 3. Assumption of Risk – I hereby expressly and specifically assume the risk of injury or harm in the activities during camp on behalf of my child and release ArtHaus Foundation, Inc. from all liability for injury, illness, death, and/or property damage that may result.

 4. Photography/Audio Release – I do hereby grant and convey unto ArtHaus Foundation, Inc. all rights, titles, and interest in and to any and all photographic images and video or audio recordings made by or on behalf of ArtHaus Foundation, Inc., or made with its consent, during my participation in its Art Camp program, and/or any project, activity or event sponsored, managed, arranged, or promoted by, or otherwise affiliated or associated with ArtHaus Foundation, Inc., including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings.

 5. Other – I expressly understand and agree that this Release is intended to be as broad and inclusive as permitted by law, and that this Release shall be governed by and interpreted in accordance with the laws of this state, county, city, and/or township. I agree that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release. By signing below, I acknowledge that I have read and understand this Release, and agree to its provisions.

 

Guidelines & Kindness Policy

PHONES & SOCIAL MEDIA: Campers may bring their cells to camp daily, however the following rules apply:

• NO SOCIAL MEDIA or YOUTUBE DURING CAMP

• Campers may use their phone for reference photos for drawing ONLY.

• During camp, the only calls or texts allowed are those to parents/guardians.

 KINDNESS POLICY: ArtHaus strictly prohibits bullying, horse-play, inappropriate use of art equipment/supplies, and the use of foul language. We use the three strike system before calling parents, however the severity of bad behavior can qualify for multiple strikes. ArtHaus administration and staff may choose to expel any camper on grounds listed above without refunds.

LUNCHES & SNACKS: ArtHaus strictly prohibits ANY form of nuts on our campus during camp, INCLUDING: peanut butter, Nutella, Kinder Eggs, Bueno Bars, etc. This is to ensure the safety of our campers & teachers with nut allergies.

Students are required to bring lunches from home and a refillable water bottle each day to camp. FAILURE TO BRING LUNCH WILL RESULT IN A $15 CHARGE/DAY FOR ARTHAUS TO PURCHASE LUNCH FOR YOUR CHILD OFFSITE. If your child will not eat snacks provided by camp, please pack their snacks for that day as well. Due to cross-contamination guidelines, lunches cannot be stored in the ArtHaus refrigerator nor can food be heated. Please pack accordingly.

ALLERGIES: If your child has an allergy not already mentioned on their registration, please notify staff immediately. All campers with allergies that require an EpiPen in case of allergen exposure must provide an EPIPEN for camp to be checked in and out daily with staff. If your camper has an  allergy to any foods provided for snack (except popsicles), please pack their snack for those days.

DRESS CODE: Please dress your child in clothes that can get messy! Please avoid dressing your child in dresses, skirts, short-shorts, or flipflops as our outdoor activities may require those dressed as such to sit out. No mid-drift shirts allowed. 

Drop-off & Pick-up: Camper drop-off each morning will be no earlier than 8:30 and pick-up no later than 5:20.

NO ONE OTHER THAN THOSE LISTED ON YOUR CHILD’S REGISTRATION MAY PICK YOUR CHILD UP FROM OUR CAMP. Pick-up any later than 5:20 each day will result in a fee to be determined by the amount of time in which you are late.

NO CAMPER UNDER 12 YEARS OF AGE WILL BE PERMITTED TO LEAVE WITHOUT AN ADULT PRESENT. CAMPERS 13+ MAY CHECK THEMSELVES OUT AT THE END OF EACH CAMP DAY ONLY IF A WAIVER HAS BEEN SIGNED BY A PARENT OR GUARDIAN.

EARLY PICK UP: PLEASE NOTIFY STAFF IF YOU NEED TO PICK YOUR CHILD UP FROM CAMP EARLIER THAN 3:35 EACH DAY.

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
