Note for Medicare: A diagnosis of diabetes or chronic kidney disease, and a referral from your doctor, are required for coverage with Medicare Part B. We do not accept most Medicare Advantage plans (Part C), except for BCBS and UHC.

Note for Medicaid (ex: TennCare/BlueCare plans): We do not accept any Medicaid plans at this time.

Onboarding fee: Before your first session, your care team spends time reviewing everything you've shared, confirming your insurance coverage, and preparing your dietitian so your first appointment is focused entirely on you -- not paperwork. The $100 onboarding fee covers that prep work. It is not billed to insurance and is nonrefundable.
