Flag Football Participation

1. Assumption of Risk
I, the undersigned, acknowledge that participation in ETTQ tryouts involves inherent risks, including but not limited to falls, collisions, sprains, strains, concussions, dehydration, heat-related illnesses, and other serious injury or death. I understand these risks may arise even without negligence by coaches, staff, or other participants. I voluntarily assume all such risks.

2. Medical Fitness & Responsibility
I affirm that I am physically fit and capable of participating in flag football. I either have received medical clearance, or I have no medical condition that would preclude participation. It is my sole responsibility to consult with a physician prior to participating.

3. Waiver & Release of Liability
In consideration of being allowed to try out for ETTQ, I release, waive, discharge, and hold harmless East Texas Football Queens (ETTQ), including its owners, board members, coaches, volunteers, staff, sponsors, facility owners, and affiliates (collectively, “Released Parties”), from any and all liability, claims, demands, or causes of action arising from injury, illness, death, or loss to myself or my property, even if caused by negligence.

4. Indemnification
I agree to indemnify and hold harmless the Released Parties from all claims, damages, defense
costs, and legal fees arising out of or related to my participation.

5. Medical Treatment Authorization
I authorize ETTQ and its representatives to seek emergency medical treatment on my behalf if needed. I understand I am responsible for all medical costs incurred as a result.

6. Media Release
I grant ETTQ permission to take and use photographs, videos, and likenesses of me during tryouts for promotional, social media, and marketing purposes, without compensation, in any form or media.

7. Conduct & Behavioral Standards
I commit to conducting myself respectfully—free from negativity, complaints, or disruptive behavior. I understand that unsportsmanlike behavior or persistent negativity may result in removal from tryouts with no refund or further opportunity.

8. Acknowledgment of Understanding
I confirm I have read and understand this waiver, and that I am giving up substantial legal rights,
including the right to sue. I sign freely and voluntarily.
