• PawPrints Spay/Neuter Assistance Program (P-SNAP) Request Form

  • PLEASE STOP and READ THIS:

    Some of our participating vets have extensive wait lists and may not be able to schedule your animal(s) for a few months. Please find out which vet can get you in the soonest before requesting your voucher. If you know the vet you want to use but are okay with waiting for an appointment from them, you may proceed.

    We will NOT issue another voucher if if you change your mind for any reason about the vet you have chosen.

  • Please be advised - because we need to limit the number of spay/neuter vouchers we issue each month, we are also limiting the number of vouchers one person can receive to TWO vouchers per month. We highly recommend getting your female animals done first to prevent unwanted pregnancies. 

    Please do not contact us requesting more vouchers if you've already received two vouchers this month. Please wait until the following month to request more, or pay for the surgeries yourself by fundraising, asking friends/family, or requesting a payment plan with the vet. Please do not make us your only option. These animals are your responsibility, not ours. We exist to help cover the cost of spays/neuters.

  • Out of our participating vets, which vet would you like to use? NOTE: By selecting a vet below, you are confirming that you have already called the participating vets to see which one best suits your needs. Once you have made your choice, we will NOT issue a voucher for a different vet for any reason. Please make your selection carefully.*
  • Are you able to pay the full *discounted* cost of the spay or neuter as listed in the previous chart?*

  • You indicated you can pay the full price at PAWS. Because they do not offer us discounted prices, that means the discounted prices you see are the prices you get. Therefore, a voucher is not needed from us. You can call PAWS directly without going through our program.

  • This program relies on people pitching in. That means, none of this is free. With that said, ​we ask that applicants for our program agree to pay at least 50% of the discounted price for spay and neuter. If you can pay more, that will allow us to help more people and animals.​

If you don't have a printer, that's okay - we can mail the voucher to you. Please make sure the mailing address you provided before is 100% accurate otherwise your voucher will not be re-sent to you if your mailed voucher gets returned to us.*

    If we have follow up questions regarding any of your responses, we will be in touch with you via email or phone. Failure to respond to our inquiries will cause your application to be denied.
