I acknowledge that cooking activities may involve the use of utensils, food, and heat sources, which carry some risks. I release and hold harmless Chef Sprouts LLC, its staff, and contractors from any liability related to injury, illness, or allergic reaction. I agree to disclose all known allergies/dietary restrictions. In case of emergency, I authorize Chef Sprouts to seek medical care for my child. I understand that photos/videos may be taken and give permission to use my child’s image for marketing unless I indicate otherwise above. I understand that if my child engages in unsafe or disruptive behavior, Chef Sprouts reserves the right to remove them from the activity without refund.
