Participant Waiver, Informed Consent, and Release of Liability I, the undersigned, understand and agree to the following terms in connection with my and/or my child(ren)'s participation in the programs and activities offered by Train Goat Gainz (Goat Kidz Program): Voluntary Participation & Assumption of Risk I acknowledge that participation in this program is completely voluntary and does not establish an employee or agency relationship with Train Goat Gainz (Goat Kidz Program I understand that some program activities may include physical movement, outdoor exploration, or other potentially strenuous or hazardous elements. I confirm that I and/or my child(ren) are physically able and have no known conditions that would prevent safe participation. I fully understand that participation may involve risks, including but not limited to: Personal injury Property damage or loss Illness, communicable disease, or exposure to viruses (including COVID-19) In rare cases, serious injury or death I accept full responsibility for these risks on behalf of myself and/or my child(ren Insurance Notice I acknowledge that Train Goat Gainz (Goat Kidz Program) does not provide any form of medical, accident, or liability insurance for participants. It is my responsibility to obtain appropriate coverage if desired. Release of Liability In consideration of being allowed to participate, I voluntarily release, waive, and hold harmless Train Goat Gainz (Goat Kidz Program), its staff, coaches, instructors, administrators, and agents from any and all liability, claims, or demands of any kind that may arise from or relate to participation in this program-regardless of cause-including ordinary negligence by other participants or staff. This release does not apply to claims resulting from proven gross negligence under applicable law. I also agree to indemnify and hold harmless the organization from any claims made by third parties as a result of my and/or my child(ren)'s participation. Media Release I give permission for Train Goat Gainz (Goat Kidz Program) to use photographs, video, or audio recordings taken of myself and/or my child(ren) during program activities for promotional or educational purposes (e.g., website, social media, flyers Names will not be published without additional written consent. Emergency Medical Authorization In the event of an emergency, I authorize Train Goat Gainz (Goat Kidz Program) and its staff to: Call 911 if necessary Arrange transportation to an appropriate medical facility Authorize emergency treatment by licensed medical professionals for myself and/or my child(ren) I understand that every reasonable effort will be made to contact me or an emergency contact before or during emergency care.
