We use encrypted storage and review practices. Your files are used only for verification. We never ask for broker passwords. Any sensitive information accidentally recieved will be protected with caution. We will never ask for confidential data. Fraudulent submissions are revoked without refund and the account will be permanently banned. 

By submitting this form you must consent to the following:

Purpose: Fiducia will use your uploaded CSV and broker statements (PDF) solely for read-only verification of trading activity and account ownership.
Security: Files are encrypted in transit and at rest. Access is restricted and logged. We never ask for broker passwords.
Data Minimization: Provide only what’s required; masked identifiers (last-4/Account ID) are preferred.
Retention: We retain verification files for up to 12 months (or less upon your deletion request), unless required longer by law or dispute resolution.
No Selling: We do not sell personal data or use it for third-party advertising.
Communication: You consent to email updates about your verification status and remediation steps.
Accuracy: You confirm the files are unaltered and exported directly from your broker for the stated period.
Eligibility: You are 18+ (or have guardian consent).
Policy Acceptance: You have read and agree to our [Privacy Policy] and [Terms of Service].
Fraud Notice: Submitting falsified documents results in permanent ban and forfeiture of fees.

Our Terms & Services and How We Protect Your Data

This quick liveness check protects you from impersonation. Your video stays private and is deleted after review.

Record a single, uncut 20–30s selfie video. Hold the handwritten note (see below) so both your face and the note are clear. Allowed: mp4, mov, webm. No edits, filters, or overlays.”

Step-by-step applicant script:

1. On a piece of paper write: today’s date, this one-time code, your name. (We provide the code on this page.)
2. Start a single, continuous selfie video (phone held vertically or horizontally).
3. Hold the note near chest level so both your face and the note are visible.
4. Say clearly, in one take:
“My name is [first and last name]. Today is [date]. My one-time code is [CODE BELOW]. This video is for Fiducia verification."

How we protect your data
• Encryption: Files are encrypted in transit (TLS) and at rest.
• Restricted access: Only vetted staff with a business need can view submissions; all access is logged.
• Data minimization: We collect only what’s needed for verification. We never ask for broker passwords, and we accept masked account identifiers (e.g., last-4).
• No selling, no ads: We do not sell personal data or use it for third-party advertising.
• Limited retention: We retain verification files for up to 12 months (or less if you request deletion), unless a longer period is required by law or to resolve a dispute.
• Trusted processors: When we use service providers (e.g., secure cloud storage), they’re bound by contract to protect your data and may not use it for their own purposes.
• Your rights: You can request access, correction, or deletion at any time.
• Breach response: If a security incident ever affects your data, we’ll notify you as required by law.

Verification Rules
1. Upload Trading History (CSV) for the last 120 days (all activity, all symbols).
2. We auto-reject if totals/fees/trade counts don’t match, or name.
3. Fraud = permanent ban across emails/devices; no refunds.

4. Export unfiltered (All activity • All symbols • Full date range). CSV only. 

*Instructions for Trading History Screen Recording* 

Record this in ONE take (no cuts):

1) Open Trading History
Go to Reports / History / Statements.
Set Date Range = last 4 months (or the exact dates you’ll claim on this form) 
Make sure these columns are visible (or closest equivalents): Date, Symbol, Side, Qty, Price, Fees, Realized P/L.
2) Slow Full Scroll (No Skips)
Sort by Date = Ascending (oldest → newest). Show the sort control briefly.
Page size = Maximum. Show the setting briefly.
Scroll rate: about 1 screen per second from the very first row to the very last row.
If there’s pagination, click through every page in order (1 → 2 → 3 …) while continuing the steady scroll if possible.
Do not use search, filters, or jump links during the scroll.

3) Export the CSV (Same Range You Just Scrolled)
Click Export / Download → CSV.
Keep the export dialog/options on screen for 2–3 seconds so we can see date range and format.
Save the file. Recommended filename format:
FIRSTLAST-UniqueID-YYYYMMDD.csv (recommended for clarity; not required).
Open your Downloads folder and show the new CSV file’s name + timestamp. Do not open or edit the file.
4) Generate the SHA-256 Hash
Open our Generate SHA-256 Hash page provided in the form.
Drag/upload the saved CSV into the tool. Wait for the 64-character code to appear.
Show the entire code on screen for around 3 seconds
