Release of Liability: In consideration of being allowed to participate in activities at Ultimate Martial Arts, I hereby release, waive, and discharge Ultimate Martial Arts, its owners, employees, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my property while participating in these activities.
Indemnification: I agree to indemnify and hold harmless Ultimate Martial Arts, its owners, employees, and agents from all claims, actions, suits, costs, expenses, damages, and liabilities, including attorney's fees, arising out of or resulting from my participation in the activities.
Medical Treatment: I consent to receive medical treatment deemed necessary if I am injured or require medical attention during my participation in the activities. I agree to pay all costs associated with such medical treatment and to indemnify and hold harmless Ultimate Martial Arts for any
Photo/Video Release: I grant Ultimate Martial Arts permission to use my likeness in photographs or videos for promotional purposes without compensation.
Property Damage: I agree to pay for any property damage I cause while I participate in any activity offered by Ultimate Martial Arts.
Acknowledgment of Understanding: I have read this waiver of liability, assumption of risk, and indemnity agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue.
