    Please complete the following form in order to provide necessary information related to your Scout(s) and family.

    Once submitted you will be directed to complete the Family Talent Survey. We kindly ask that you complete BOTH surveys. 

     This form is to be completed EACH YEAR by one Parent/Guardian for currently registered Scouts of Pack 444.

    This form is required for both new and returning Scouts. 

  • General Family and Scout Information

    Please provide the following information as it relates to you and your Scout(s) family. This information will help us better contact you as needed and also ensure we have critical information when it comes to working with your Scout(s). This also provide us with your Scouts Class B T-Shirt size which is necessary

  • Provide Class B T-shirt sizes for your Scout(s) using table below.
    List each Scout on separate row. Even if your Scout currently is not due for a new Class B T-shirt its helpful for us to know what sizes may be needed soon. 
    Sizing should be based off Gildan Brand 100% Cotton T-Shirt.

    Note: The Pack provides each Scout at no cost 1 Class B T-Shirt every two years. Therefore, keeping this in mind when choosing size is beneficial as T-Shirt should follow the 2-year cycle. It may be ideal to choose 1 size bigger than what your Scout is currently wearing if your Scout grows out of things quickly or is currently in a growth spurt. Additional shirts may be purchased should replacement be required prior to 2-year cycle.

  • Please use the box below to inform us if your Scout(s) have any Dietary Restrictions/Food Sensitivities, Allergies (requiring use of EpiPen) or Major Health Conditions we should be aware of.
    Even though our Pack requires a parent be present with their Scout at all Pack related activities including weekly meetings at times its maybe necessary for us to be aware of any of the above items. This will also help us ensure we create a safe and inclusive environment when planning events that food may be present as well as physical activities that may occur throughout the year.
