Acknowledgement of Risk
I acknowledge that participation in soccer trials, training sessions, and related activities with Moorebank Sports Soccer Club (MSC) involves inherent risks, including but not limited to:
*Risk of injury from physical contact, falls, or collisions
*Environmental risks such as weather or field conditions
*General risks associated with physical activity and sport
*I understand that while MSC takes reasonable steps to provide a safe environment, the Club, its coaches, officials, and volunteers are not liable for any injury, loss, or damage that may occur during participation in trials.
Medical Consent
*I confirm that I (or my child) am medically fit to participate in soccer trials.
*I agree to disclose any relevant medical conditions or injuries to the Club prior to participation.
*In the event of injury or medical emergency, I authorise MSC officials to provide or arrange medical treatment as required.
*I accept full responsibility for any associated medical costs.
Insurance & Liability
I understand that trial players are not covered by MSC player insurance until they have formally registered and paid with Football NSW.
Participation in trials is at my own risk.
Code of Conduct
I agree to:
Respect coaches, officials, and other players at all times
Follow all instructions and safety guidelines
Uphold the values and reputation of Moorebank Sports Soccer Club
