I acknowledge by signing this release that I have been given the full opportunity to ask any and all questions which I might have about obtaining tattoos from the Technician Karissa Lau and that all my questions have been answered to my full and total satisfaction.
Procedure to be performed: Fine line and tiny tattoo
I specifically acknowledge that i have been advised of the matters set forth below and agree as follows:
I certify that I am over the age of 18 years old and that I have a U.S. government issued I.D. to prove so. I acknowledge that obtaining a tattoo is my choice alone. I understand that the application of tattoo will result in a permanent change to my appearance.

Post-Procedure Aftercare
• Keep wrap on for 2-4 hours. Remove and wash and clean.
• Always wash with cold water and clean with unscented soap thoroughly and gently.
• Always apply a tattoo balm such as
Hustle Butter after every wash.
• Repeat these steps until the tattoo is fully healed. This may take 2-4 weeks.
• Avoid tight clothes on the tattoo area such as bras and jeans
• Avoid sun and dirty environments.
• Avoid going to the pool and beach
• Do not pick the scabs
Please acknowledge the following:
I understand that the tattoo site usually takes 2-4 weeks or longer to heal and that every procedure may require two sessions at least 4 weeks apart, to achieve the desired look. I acknowledge that the results of my tattoo varies on several factors such as skin, aftercare, lifestyle choices, sun exposure, etc. and that it is my full responsibility to follow the aftercare for the best results. I acknowledge that results varies and that it may not uphold my expectations. I have received pre-and post-procedure instructions and I will strictly adhere to such instructions. I understand that my failure to do so may jeopardize my chances for a successful procedure and healing results.
I agree to release and forever discharge, and hold harmless, the Technician, all employees, contractors, and the management of the permanent make-up /tattoo studio from any and all claims of negligence, damages, or legal actions arising from or connected in any way with my tattoo, the procedure, and conduct used in my tattoo and assume all responsibility for the decision(s) made consenting to this permanent procedure.
I acknowledge and give consent to this tattoo studio to use images of my tattoos) for marketing and, or publishing purposes in various media such as the internet, magazine, printed, and/or television etc. I consent to allowing photos and videos taken of me before, during, and after the procedure..
I acknowledge that the Technician has the right to refuse service to any client who he/she deems unfit for the procedure. I acknowledge Beautific Studios LLC policies and agree to adhere to all company and appointment policies.
I certify that I have read and fully understand the contents of this form. I understand that this consent applies to all my future appointments. I understand the risks inherent in the procedure and have possibilities of complications during and/or following the procedures such as: infections, misplaced pigment, poor color retention and hyperpigmentation. I authorize the technician as my permanent cosmetic technician to perform on my body.
