Theatre Kids – Consent & Waiver Form
1. Assumption of Risk & Liability Waiver

I, the undersigned, acknowledge that participation in Theatre Kids classes, rehearsals, and performances involves certain risks, including the possibility of accident or injury.

I agree to assume all such risks on behalf of my child. I release and hold harmless Colleen Aynn Borges, Theatre Kids, its instructors, staff, associates, volunteers, and affiliates from any and all liability, claims, or demands that may arise from my child’s participation, except in cases of gross negligence or willful misconduct as defined under Ontario law.

I understand that instructors will take reasonable steps to provide a safe environment and to prevent accidents or injuries.

2. Photo/Video Release

I grant permission for Theatre Kids to photograph and/or record my child during classes, rehearsals, and performances. I understand these images or recordings may be used, reproduced, and shared for promotional, advertising, educational, or archival purposes in any format (print, video, or digital media) at Theatre Kids’ discretion, without compensation.

I waive any right to inspect or approve the final materials and release Theatre Kids and its affiliates from all claims arising from the use of my child’s likeness.

3. Acknowledgement

I confirm that I am the parent/legal guardian of the child named below and that I have the authority to provide this consent. I acknowledge that I have read, understood, and voluntarily agree to the terms above.
