Participation/Media Waiver and Release
Please read the following information carefully before signing your name.In exchange for the opportunity to work on, be involved with, or perform in theatrical productions at Heart House Inclusive Productions Inc., the undersigned hereby releases and waives all rights and claims against the Theatre for all losses, damages and injuries of any kind, regardless of how they may have arisen. It is understood that there may be risks involved in working in the Theatre whether on stage or off and that these risks might cause losses or injuries of some kind. Recognizing that risks may exist, I none-the-less freely, knowingly and voluntarily sign this release and waiver.I hereby attest that this waiver of liability is provided voluntarily upon submission of this form and shall be fully binding upon me, my heirs, next of kin, executor, administrator and/or personal representative. Further, in consideration of Heart House Inc granting permission to participate, I hereby grant permission to use my/our child(ren)’s name and image in any publicity whether by print or electronic means for the purpose of informing the community of events, publicity, advertisement or production sales or distribution. This includes, but is not limited to newspapers, magazines, radio, television, the internet, social media, as well as publicity for the Theatre. This release and waiver expressly includes Heart House Inclusive Productions Inc., its employees, volunteers, agents, sub-contractors, and assigns.

Heart House is committed to creating and maintaining a respectful and inclusive community, all Heart House performers and volunteers agree to the following code of conduct: RESPECT; I will act in ways that bring respect to me, my cast, my crew, and the Theater. I will not insult other persons at Heart House rehearsals or performances. I will not fight with other cast or crew. My language, manner, punctuality, preparation and presentation will demonstrate high standards. REHEARSALS AND PERFORMANCES; I will practice my lines and music outside of rehearsals so I am prepared by designated dates. I will listen to my director and production staff and be respectful when asking questions or raising concerns. I will treat everyone equally regardless of sex, sexual orientation, ethnic origin, religion or ability. I will be a positive role model for other members of the organization. RESPONSIBILITY FOR MY ACTIONS; I will not make inappropriate or unwanted physical, verbal or sexual advances on others. I will not smoke or chew tobacco at rehearsal/performance space except in designated smoking areas. I will not consume alcoholic beverages and/or controlled substances during any Heart House event.I will not engage in sexual activities during any Heart House rehearsals or performances. I will respect the property of Heart House, the Ferlazzo Building, and others.

Waiver and Release of Liability, Assumption of Risk and Indemnification Agreement for Communicable Diseases
Please read the following information carefully before signing your name. In consideration of being allowed to participate in any way in Heart House Inclusive Productions Inc., the undersigned acknowledges, appreciates, and agrees that: 1. Participation includes possible exposure to and illness from infectious and/or communicable diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and, 2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, 3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, 4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Heart House Inclusive Productions Inc., their officers, officials ,agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
