Informed Consent and Acknowledgement
I hereby give my approval for myself and/or my child(ren) to participate in any and all events and activities prepared by MilestoneZone LLC. In exchange for participation, I assume all risks and hazards incidental to the activities and release, absolve, and hold harmless MilestoneZone LLC, its officers, agents, representatives, and affiliates from any and all liability for injuries or damages that may occur while traveling to, participating in, or returning from any such events or activities.
In the event of an injury, I waive all claims against MilestoneZone LLC, including its staff, volunteers, guest speakers, sponsors, advertisers, and, if applicable, the owners and lessors of the premises used for these events and activities. I acknowledge that participation in any physical or hands-on activity carries inherent risks, which may include, but are not limited to, the risk of fractures, paralysis, or other serious injuries.
Alcohol Policy Acknowledgement:
I understand that certain events may allow alcohol consumption on a BYOB basis or through licensed service. I acknowledge that I am solely responsible for my own alcohol consumption and actions. If I choose to consume alcohol, I agree to do so responsibly and in compliance with all applicable laws. I further release MilestoneZone LLC from any liability related to the consumption of alcohol by myself or my guests.
