Permission to Release Information – Teacher Recommendations
Instructions for Parents: Complete, sign, and return to Nexus Futbol Academy.

Please complete and sign this release form before we contact the teachers you identify below. We encourage you to notify your child’s teachers that a recommendation request will be coming from Nexus Futbol Academy as part of the admissions process.

Your signed release authorizes the teachers to share information that will help us better understand your child, including academic performance, classroom engagement, and overall development.
