Permission to Release Information – Coach Recommendation
Instructions for Parents: Complete, sign, and return to Nexus Futbol Academy.

Please complete and sign this release form before we contact the coach you identify below. If your child has recently changed clubs or teams, you may list a coach from the past year.

We encourage you to notify your child’s coach that a recommendation request will be coming from Nexus Futbol Academy as part of the admissions process.

Your signed release authorizes the coach to share information that will help us better understand your child as a student-athlete, including observations about character, teamwork, and overall development.
