What You Need to Pay:
Below is the list of fees associated with reinstating your ACLS Licence to Practice. To complete your application, you must select and pay all applicable fees.
Use these tips to guide your choices:
Did you file a DECLARATION OF CHANGE IN MEMBERSHIP AND/OR LICENCE STATUS (previously called Retiring CLS Checklist) by December 31 in the year your licence expired?
If yes, you are required to pay Fee 1 & Fee 2 only.
If no, you are required to pay Fee 1, Fee 2, Fee 3, Fee 4.
Fee 1 - Start with the base reinstatement fee — depending on how long your license has lapsed, you must check one box to add the appropriate fee to your cart. Anyone reinstating their Licence to Practice must pay this fee.
Fee 2 - One year Licence to Practice Fee (for the current year) will be automatically added to your cart.
Fee 3 - This is a penalty you are required to pay if you did NOT complete the DECLARATION OF CHANGE IN MEMBERSHIP AND/OR LICENCE STATUS (previously called Retiring CLS Checklist) by December 31 in the year your membership or licence expired.
Fee 4 - If your Licence to Practice has lapsed 12 months or more, you are required to pay up to three years in back-fees. If applicable, choose the number of years your licence was inactive and click the appropriate box to add the fee to your cart.
Please be sure that you have reviewed each of the 4 (four) fee sections below and selected all the appropriate fees for your situation. The four sections can be collapsed or expanded using the arrow on the right of each fee section.
Review the “Total” field carefully — it will sum up all your selections to show what you owe.
