Character Education Now, Inc, Parental Consent Form

I am the legal parent/guardian of (Child's full name) and I desire to enroll my child in the school's character and success based instructional program. This program is a story time utilizing PowerPoint presentations read to the class to encourage good character. For the pre-K to 2nd grade students, we present the Chip series in which Chip and his friends learn valuable lessons. Here are five examples of the lessons:

By signing below, I acknowledge the character and success-based nature of the school's instructional program and consent to my child's enrollment in this program and related activities. I agree to comply with all attendance, conduct and other policies applicable to students enrolled in the program, and I agree to support my child's successful character development and maintain open communication with school personnel. This consent shall be effective for the 2025-2026 school year.
