All uploaded documents must EXACTLY match the name and address provided at the time of the original report. No exceptions. No discrepancies. No excuses.
Attorney letters verifying your claim are permitted without address matching, but they must be legitimate and specific to the content in question.

Cease and desist letters will NOT be accepted through this form. If you have one, it must be emailed directly to the address listed on our official Facebook page.
Attempts to bypass these rules or pressure our team will be ignored.
