Bright Times, LLC (The Nook) Admission Agreement
On behalf of myself, my spouse (if applicable), and each child I am enrolling (my “Child”), I enter into this Admission Agreement (“Agreement”) with Bright Times, LLC, a Nebraska Limited Liability Company, which is an independently owned and operated.
Your active registration allows you to visit any participating center, as long as you have visited at least once every 12 months from your original enrollment date.
Facility Use
Under this Agreement, your Child may participate in drop-in, short-term child care at The Nook on a varying schedule. This includes use of our play areas and participation in activities. Full-time or reserved part-time care or private use require additional agreements. The Nook does not take field trips or provide transportation.
Future Visits
This Agreement, along with the Registration Form and Release, will be kept on file for at least one year past your last visit date. It will continue to apply to any future visits to any Nook center as long as you visit once every 12 months from your last visit date.
Please note that this Agreement does not obligate The Nook to provide services indefinitely, and The Nook reserves the right to decline admission to any child at any time, without liability.
Payment
Payment is due at check-out for each visit. Payment for full-time care or reserved care may be required in advanced as outlined in related agreements. Charges for drop-in care are based on the time from check-in to check-out, multiplied by the posted hourly rates. Additional posted fees may apply for services such as family registration, meals, or retail items. Unpaid balances may be subject to service fees. No refunds are provided. Any changes in fees will be posted at least 30 days in advance.
Medical Procedures
First Aid & Procedures: While The Nook provides a safe environment, I understand that injuries may occur. I authorize The Nook staff to provide reasonable first aid, following internal procedures, while allowing them discretion to act as circumstances require.
Safety & Indemnity
The Nook may take whatever action it deems appropriate to ensure the safety of my Child and other children in the facility.
I agree to indemnify and hold harmless The Nook, its franchisees, and all associated owners, officers, directors, agents, and employees from claims, actions, or liabilities (including attorney fees and court costs) resulting from: my Child’s actions, or any inaccuracies or omissions on my Registration Form.
Nebraska Licensing Requirements
As required by the State of Nebraska, the licensing agency has the authority to:
- Interview children or staff,
- Review child and facility records,
- Observe children’s physical condition (including signs of possible abuse, neglect, or inappropriate placement), and
- Have children examined by a licensed medical professional.
Additional Requirements
As a condition of service, I confirm that I have accurately completed and signed the Registration Form and Release. The Nook will rely on this information in caring for my Child. I agree to pay all costs and attorney fees resulting from any legal action related to this Agreement, the Registration Form, or the Release.
