Our Agreement
To provide you with the best experience, please read the below terms and conditions carefully.
By completing this application, I confirm that I am available and will be in attendance in full at all the indicated sessions. I will notify the Interdisciplinary Institute via email or in-person as far as possible in advance if I am not able to attend.
Please Note: Doors at 5pm, Session from 5:15-6:15pm. No Late Arrivals after 5:15pm.
Cancellations and Rescheduling:
All services during the selected teaching/consulting period are non-refundable upon completion of this form and payment of the administrative fee, signifying enrollment. All terms apply.
All sales are final with no refunds.
Session presenter lineup is subject to change with at least 24 hours notice to participants.
If in the case any sessions are not able to be conducted at the desingated time due to unforeseen factors beyond our control, we will provide participants with an alternative rescheduled time for which they can use their session passes at.
Purchases made through this form may only be applied for this program "Sound Bath Sundays" and none other.
If participant is not able to attend a session, with at least 24 hours notice (unless provided with a doctor's certificate), they may use their pass for a different session within the listed "Sound Bath Sundays" dates, or gift it to someone else for use. This must be communicated via email to info@interdisciplinaryinstitute.com. Cancellations within 24 hours will not be able to be transferred due to our in-demand sessions, in order to open up spots for other potential participants.
Privacy
This form and its contents is for private use between the above listed parties only (the student/client and Interdisciplinary Institute LLC) and shall not be shared or distributed in digital format or the specific contents within this form without direct consent from Dr. Joy Yang and/or the client or student unless otherwise indicated. This form and any accompanying document(s) contain information for the sole use of the above-intended recipient(s) and may contain privileged or confidential information. Any other distribution or use of this communication is strictly prohibited.
Clients and students may, however, inform the public about the benefits and existence of this program, our enrollment tracks, enrollment periods or dates, share publically available material such as our webiste and social media, and connect them with the Interdisciplinary Institute. Parties are welcome to check with Dr. Joy if they are unsure.
The student/client agrees to Interdisciplinary Institute LLC's public acknowledgement of the student/client being enrolled in our program whenever necessary (unless otherwise indicated directly via writing to info@interdisciplinaryinstitute.com).
Participation Waiver And Release
As the legal parent or guardian of the student listed on this form, or the stated student/client participant myself, I understand that my or my child’s participation in the 1-1 consultations and/or lessons and sessions through the Interdisciplinary Institute LLC’s "Sound Bath Sundays" Program may expose him/her/them/myself to unforeseen risk of personal injury or property damage. I am aware of the dangers of attending this program during my enrolled teaching/consultation period. I hereby acknowledge that I am voluntarily allowing my child/myself to participate in lessons and/or consultations and agree to assume any such risks.
I promise not to hold “the company” (also referred to as “Interdisciplinary Institute LLC”, or “Dr. Joy Yang”), its officers, agents, teachers, contractors, or employees liable or pursue legal action against them. This waiver absolves the company of any duty for injuries sustained on the premises before to, during, or after the activity.
I hereby release, discharge, and agree not to sue the “the company” and its agents or employees for any injury or damage to or loss of personal property arising out of or in connection with my or my child’s participation in lessons from whatever cause, including the negligence of private lessons instructors or any other participants of the Interdisciplinary Institute LLC.
I hereby agree that I shall indemnify and hold harmless “the company” from any and all claims, demands, actions, or suits arising out of or in connection with my or my child’s participation in lessons and/or consultations.
I affirm that I have read, understand, and agree to be bound by the term and conditions of this contract for 1-1 lessons and/or consultations through the Interdisciplinary Institute Private lessons and Consultations Program .
By signing this agreement, I agree to hold the company completely harmless, including financial responsibility for any injuries sustained, regardless of the cause or circumstances. I will do all in my power to obey and cooperate with staff and all safety requirements, and I will seek clarification if necessary.
I have read and fully understand the above important information, warning of risk, waiver and release of all claims, and assumption of risk. If registering online or via fax, my online or facsimile signature shall substitute for and have the same legal effect as an original form signature. This Waiver & Release is effective for all private lesson or consultations taken on and after the date signed.
