Before we begin your permanent jewelry service, please read and sign the following information to ensure a safe and smooth experience.
Service Acknowledgment
Permanent jewelry involves welding a small jump ring to close your selected chain. This process may produce minimal heat and small sparks, both are completely normal and generally harmless. At MxBeautyCo we use professionalgrade tools and safety precautions to minimize all risks.
While this is considered a safe and lowrisk service, minor possibilities such as brief pinching or temporary discomfort may occur. By signing below, you acknowledge and accept these risks.
Jewelry Care & Maintenance
All jewelry, regardless of material, is delicate to some degree. Over time, chains may snag, stretch, break, or tarnish due to normal wear or exposure to elements.
MxBeautyCo kindly provides a 30-day warranty, free re-welds and sizing can be done in this period.
If the jump ring breaks, a re-weld fee of $10 applies per weld.
If the chain itself breaks or is lost, a new chain purchase is required.
Once your chain is welded, the sale is final — no refunds or exchanges.
If sizing adjustment is needed a $10 re-weld fee applies per weld.
***Tarnishing Notice:
.925 Sterling Silver and 14k Gold Filled jewelry may discolor or tarnish due to factors such as skin pH levels, sweat, perfumes, lotions, or hair products. This is a natural reaction and not considered a defect MxBeautyCo is not liable for such defects. Cleaning wipes are provided at each appointment, please care for you forever welds.
Health & Safety
**Clients with Pacemakers should not receive permanent jewelry due to the welding process.
Please inform your provider, Michelle, of any skin sensitivities, allergies, or medical conditions that may affect your service.
Jewelry Removal
If you ever need to remove your jewelry:
Cut the jump ring only using scissors or wire cutters.
Cutting the chain itself will void eligibility for a re-weld and require a new purchase.
Re-welds are available for $10 per weld by appointment only.
Understanding of Risks and Procedure:
I acknowledge that I have discussed the procedure and its potential risks with the practitioner Michelle . I understand that the procedure may involve:
- A small shock or pinch
- Long-term care and maintenance requirements for the jewelry.
I have been provided with aftercare instructions, and I understand the importance of following them to minimize complications.
Consent and Release:
I voluntarily and willingly consent to the permanent jewelry procedure and assume any risks associated with it. I release MxBeautyCo, its practitioners, employees, and affiliates from any liability for complications, injuries, or damages resulting from the procedure.
Age Verification:
I confirm that I am at least 18 years of age, or if I am a minor, I have obtained the consent of my parent or legal guardian for this procedure.
Photo Release:
I give permission for photographs or images of the procedure and the jewelry placement to be taken for documentation, content and educational purposes.
Signature:
I have read and understood this Permanent Jewelry Consent Form, and I sign it of my own free will.
