  • The Community Health Worker Collaborative of South Dakota (CHWSD) is excited to introduce a sponsorship opportunity to sponsor two conferences in 2026:

    • 5th Annual South Dakota CHW Conference | April 29 and 30, 2026 | Mitchell, SD
    • 12th Annual Fall Forum - A Conference Co-sponsored by the CHWSD and SD Health Link | October 14, 2025 | Sioux Falls, SD

  • Single Conference Sponsorship Levels

    Select which conference and level you would like to sponsor.

  • BOTH Conference Sponsorship Levels

    Select which level you would like to sponsor for both conferences.

  • Company/Organization Summary and Logo

    Sponsor logos and program ads are due to CHWSD by March 15, 2026, at the latest for the 5th Annual Conference, and by September 15th for the 12th Annual Fall Forum to ensure printing prior to the conference.
