I agree to provide a safe, loving, and temporary home for any dog/puppy placed in my care by Forever Kuddles Fosters. I understand that all medical care, food, and supplies will be provided by the rescue unless otherwise
agreed. If I can no longer care for the foster dog/puppy, I will immediately contact the rescue for a safe transfer. I understand that falsification of any information may result in immediate removal of the foster dog/puppy.
I also understand that puppies who are not fully vaccinated (under 16 weeks of age) must not be taken to public
dog parks, pet stores, or other high-risk environments where exposure to unvaccinated dogs may occur. These puppies are especially vulnerable to contagious and often deadly diseases such as Parvovirus, Distemper,
Kennel Cough, and Canine Influenza. They must be kept away from unknown dogs until they receive their third set of vaccines; additional guidance can be provided by Forever Kuddles Fosters if needed.
If a foster dog or puppy develops a health issue, I will contact Forever Kuddles Fosters immediately for direction on where to seek veterinary care. Forever Kuddles Fosters works with approved veterinarians, and unauthorized vet visits may not be covered.
