Our training programs sell out VERY QUICKLY. This is a committment to the training program. Our Terms and Conditions are non-negotiable at any time. We've designed the terms to ensure a fair experience for all participants and coaching staff.

Unforseen circumstances are unfornate but they do happen (i.e. injuries, illness, etc). This is why we've included the option for Cancellation Insurance and to help provide some peace of mind. Please read it carefully and take into consideration.
PLEASE ACKNOWLEDGE: Unless the Cancelation Insurance is purchased WITH THIS REGISTRATION, I understand the following policy: (1) Training session fees are NON-REFUNDABLE; (2) Fees CANNOT be moved to another session.*

(1) INFORMED CONSENT & DISCLAIMER:

During any training, camp, or clinic, every effort will be made to assure athlete safety. However, as with any training program, there are risks. These risks may include, but not limited to, the chance for musculoskeletal injuries & stress on the cardio-respiratory systems. By approving this consent & disclaimer, you assume the responsibility for these risks and waive any possibility for personal damages. You also agree that, to your knowledge, you have no limiting physical condition(s) or disability that would preclude you from this goalkeeper training program. If you do have any medical issues or condition(s) that may be of concern, you further agree that a physician has cleared you to participate in this type of training program. Participants who do not have prior physician clearance MUST acknowledge they have been informed of its importance. By consenting below, you accept full responsibility for your own (athlete’s) health & well-being as stated above and you acknowledge an understanding that the leaders of this program, as well as the facility administration (i.e. Colorado Academy, Golden Goal, Foothills, etc.), assume no responsibility.

Please note that the local goalkeeper coach is NOT in control of the training facility and grounds in which trainings are held and, therefore, cannot be held responsible for any incidents within the facility or on the grounds. However, all coaches will do what is possible to maintain safety within their training area(s).

By approving this document, I hereby authorize Zuriel Lozano (GK Coach - ZPro Futbol) to use my [goalkeeper] name and likeness, voice, verbal statement, and filmed media (photos or video) for any of the aforementioned purposes.

OPTIONAL ... this is our "Peace-of-Mind" insurance option! Due at the time of registration. This option allows for a refund of the fee paid for the training session that was paid with this registration. The refund may be prorated based on when the cancellation is requested: full refund if cancelling more than 10 days prior to the start of the training; 75% refund if cancelling 4 to 10 days prior to the start of training; or, if cancelling less than 4 days prior to the start of the training session then only a credit (no refund) will be given that must be used within 60 days of the end of the 4-wk session. --This insurance is for CANCELLATION of the entire registration only... NOT for missing single (or more) training days.-- *THIS CANCELLATION INSURANCE FEE IS COMPLETELY NON-REFUNDABLE*
