PLEASE READ THIS CAREFULLY. It affects any rights you may have if you and/or your dog(s) are injured or otherwise suffer damages while attending a Michigan Performance Dog Club ("MPDC") event.

This Release and Waiver is intended to shift the risk of participating in MPDC-hosted events to the Owner(s)/Handler(s) of the dog(s) in the events. Do not sign it unless you are willing to accept assumption of the risk of participation and the risk of bodily injury, death or property damage while at a MPDC-hosted event and upon the property where any such event is held.

I (we) agree that MPDC has the right to refuse this entry. In consideration of the acceptance of this entry and of the holding of any such event and of the opportunity to have the dog judged and to win prizes, ribbons, or trophies, I (we) agree to hold MPDC and/or any sponsors, their members, directors, officers, agents, and event secretary and the owner(s) and/or lessor(s) and/or lessee(s) of the premises where such event is held and any provider of services associated with such event and any employees or volunteers of the aforementioned persons/entities, and any MPDC-approved judge officiating at this event, harmless from any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of this dog while in or about the event premises or grounds or near any entrance thereto, and I (we) personally assume all responsibility and liability for any such claim; and I (we) further agree to hold the aforementioned persons/entities harmless from any claim for loss, injury or damage to this dog. Additionally, I (we) hereby assume the sole responsibility for and agree to indemnify, defend and save the aforementioned persons/entities harmless from any and all loss and expense (including legal fees) by reason of the liability imposed by law upon any of the aforementioned persons/entities for damage because of bodily injuries, including death, at any time resulting therefrom, sustained by any person or persons, including myself (ourselves) or on account of damage to property, arising out of or in consequence of my (our) participation in any such event, however such injuries, death or property damage may be caused, and whether or not the same may have been caused or may be alleged to have been caused by the negligence of the aforementioned persons/entities or any of their employees, agents, or any other persons.

All risks are known and appreciated by me. I (we) waive any and all specific notice of the existence of the risks. I (we) shall assume sole responsibility for and pay my (our) and my (our) dog's/dogs' medical and emergency expenses in the event of injury, illness or other incapacity regardless of whether I (we) authorized such expenses. I (we) agree that I (we), as owner(s) of the above dog(s), am solely responsible for the dog's (dogs') behavior and physical control and agree to carefully monitor and control at all times.

This Release/Waiver shall survive the conclusion of any event to which this Release applies.

This agreement supersedes any prior, contemporaneous oral or written agreements or negotiations. This agreement may not be modified except through written agreement signed by the dog owner(s)/handler(s) and MPDC.

MPDC is not responsible for any errors on the entry form nor are we responsible for any errors made by the exhibitor(s) using the corresponding entry forms when entering events.

I (we) confirm that I (we) have read, understand and agree to the above release/waiver. I (we) hereby agree that this is my (our) legal and binding typed signature(s I (we) am (are) the owner(s) or agent(s)/handler(s) duly authorized to make the corresponding entry and complete this Release/Waiver.
