Once you've filled out this form, please submit payment through either Venmo or Chase Quickpay. PAYMENT IS DUE IMMEDIATELY AFTER PLACING YOUR ORDER.

Cookie pickups will be made from my house in Deerfield. I will send you the address when I confirm receipt of your payment. Pickups will be on either Friday, October 30 OR Saturday, October 31 from 8AM-6PM, please be sure to select your preferred pickup date below.
