Please complete the following form to be considered for an educational booth at the upcoming April 29 & 30, 2026, SD CHW Conference in Mitchell, SD and the October 14, 2026, 12th Annual Fall Forum - A Conference Co-sponsored by the CHWSD and SD Health Link. Please only consider submitting a proposal for an educational booth if your organization provides education to CHWs/CHRs and/or provides regional or statewide resources. Notice of selection will be provided by March 1, 2026, for the April conference and September 1, 2026, for the October conference to allow organizations adequate planning for the event.
An option will be given below to have a booth at one conference (select which one) or for both conferences.
- The fee for non-profit organizations is $350 for one conference or $700 for both conferences (includes 2 attendee registrations for each conference).
- The fee for for-profit organizations is $500 for one conference or $1,000 for both conferences (includes 2 attendee registrations for each conference).
Organizations will be asked to provide a door prize with a value of ~$50. Attendees will receive a passport to have booths sign off on to be eligible to participate in the door prize drawing.
