Inventory Transfer Form

This form must be completed before removing any inventory item. Items cannot leave the clinic until the inventory and/or drug log have been updated.
Please note: Only doctors can transfer controlled substances. If a doctor is transporting controlled substances, it needs to be notated on the drug log and both doctors must sign off on the transfer. It is a felony for anyone other than a doctor to transport any controlled substance.
