  • Has your organization had your IRS 501(c)(3) Letter of Determination for at least 2 years?
  • Does your IRS 501(c)(3) Letter of Determination address match your current 990 address or would you be willing to update it?
  • Do you have a current Certificate of Continued Existence from the Ohio Secretary of State?
  • Is your registration current with the Ohio Attorney General?
  • Does your organization have a current City of Columbus Solicitation Permit? If not, are you willing to get one?

  • If affiliated with a national/state organization, do you have a letter of local presence or is your headquarters willing to write one?
  • Is your board composed of at least 3 voting, uncompensated members with no material conflicts of interest?

  • Does your organization follow Generally Accepted Accounting Principles (GAAP)?
  • Which 990 have you filed with the IRS within the last 18th months?
  • If you file a 990-EZ or 990-N, you must provide completed pages 1, 9, and 10 of the full 990. Are you willing to do this?
  • According to your most recent IRS Form 990, are your organization’s combined administrative and fundraising expenses (FRA) 30.0% or less of total revenue?
  • If your FRA exceeds 30%, can you provide a formal written plan explaining why the costs are reasonable and how you plan to reduce them? If your FRA is lower than 30%, answer yes.
