Please provide a brief description of your current coaching & mentoring practice. You may wish to include:
• Any areas of specialism you have developed or are developing
• Whether you work with a specific client group (e.g., professionals, young people, community groups)
• The type of coaching/mentoring you typically deliver (e.g., one‑to‑one, group, organisational)

2.1 Certified Training Courses 

Please detail the training courses you have undertaken that are directly related to coaching & mentoring. Applicants must demonstrate a minimum of 50 hours of training delivered by UK-based providers. This training must include live, interactive (synchronous) teaching, observed practice with feedback, study of coaching and mentoring theory, structured skills practice, and practitioner reflection and review.

Coaching & Mentoring experience hours

Please provide details of your coaching & mentoring practice that demonstrate you meet the membership requirement of:

• A minimum of 50 hours of coaching & mentoring delivered
• Over a period of 3 years
• With a minimum of 5 individual clients

Please use this section to introduce yourself and share what being a coach or mentor means to you.

Provide a brief overview of your background, including:

• What is your philosophy of coaching & mentoring
• How long you have been practising
• Any specialisms you have developed or are developing
• The client groups you work with
• Organisations or settings where you have gained experience
• Why you would like to be a member of the ACC coaching & mentoring community

6.1 Christian Sponsor

A Christian sponsor is someone who can confirm that they know you to be a Christian. We will not be asking them to comment on the nature or strength of your faith or church attendance. 
 
A sponsor is normally someone in church leadership but can be someone else if you are between churches for example. We ask that your sponsor is not a family member or close friend and that they have known you, ideally for a minimum of two years. 
 
We email sponsors and ask them how long they have known you and in what capacity. We ask them to confirm that, to the best of their knowledge, you are a Christian and whether they would have any concerns about you becoming a member of ACC. 
 
Please ensure that you have explicit consent to record your sponsor’s name and contact details as requested on this form. 

6.2 Christian sponsor reference

Please send the following online form to your Christian sponsor using the link provided.

Please note it is your responsibility to ensure that the Christian sponsor reference is submitted to ACC prior to, or at the same time as, submission of application form. Applications will not be processed without a Christian sponsor reference being received.

7.1 Professional Referee

A professional referee can be your supervisor or a colleague who knows your practice and is an accredited coach or otherwise has sufficient training and experience to qualify as an ACC verified coach & mentor. We will contact them to ask how long they have known you, in what capacity and whether they support your application to be a verified coach with ACC.  
 
Please ensure that you have explicit consent to record your referee’s name and contact details as requested on this form. 

7.2 Professional reference

Please send the following online form to your professional referee using the link provided.

Please note it is your responsibility to ensure that the professional reference is submitted to ACC prior to, or at the same time as, submission of application form. Applications will not be processed without a professional reference being received.
