By signing this form, you confirm that all information provided is accurate and truthful. Any false or misleading information may result in immediate dismissal or disqualification from the pageant without refund.
All deposits and fees are non-refundable and cannot be transferred to another person. However, payments may be credited toward a future event if cancellation notice is given at least 7 days prior to the scheduled pageant.
By signing below, you acknowledge that you have read, understand, and agree to all pageant rules and guidelines. You also grant Dreamtime Works LLC permission to use your child’s photos for advertising and promotional purposes.
Age and level verification must be available on the day of the pageant if requested by staff.
You further acknowledge that cash prizes are guaranteed only when the minimum required number of contestants is present.
