Ages 5-14 please join us for a fun-filled wrestling camp on December 30-31, 2025! Prepare to learn fundamentals, participate in games and live matches, and receive a free T-shirt. Location Jenkins Middle School Gym. 9:00am-1:00pm. Lunch will be from 11:00-11:30. Please bring a lunch as only a light snack will be provided. Please fill out the form below to register!

WAIVER AND RELEASE OF LIABILITY

By signing this form, I acknowledge and agree to the following:

    1    Voluntary Participation:

I understand that participation in the Chewelah Mat Cats Wrestling Club Camp (the "Camp") is voluntary and that my child is participating at their own risk.

    2    Assumption of Risk:

I understand that wrestling and related physical activities carry inherent risks of injury, including but not limited to sprains, strains, fractures, concussions, and other serious injuries. I assume full responsibility for any and all risks of injury, illness, damage, or loss that may result from participation in the Camp.

    3    Release of Liability:

I hereby release and hold harmless the Chewelah Mat Cats Wrestling Club, its coaches, volunteers, organizers, affiliates, and any other associated parties from any and all liability, claims, demands, or causes of action arising out of or related to participation in the Camp.

    4    School District Disclaimer:

I understand and acknowledge that the school district is not affiliated with or responsible for the Chewelah Mat Cats Wrestling Club Camp and shall not be held liable for any injuries, losses, or damages that may occur during participation in the Camp.

    5    Parental/Guardian Supervision Required:

I understand that a parent or legal guardian must remain present with their athlete at all times during the Camp. No athlete may attend without a responsible adult present for the duration of the event.

    6    Medical Authorization:

In the event of an emergency, I authorize the Camp staff to secure appropriate medical treatment for my child. I agree to be financially responsible for any medical treatment or services provided.
