  • MELITA'S DREAM ANIMAL RESCUE PET CARE AGREEMENT

  • The purpose of this Authorization and Agreement is to provide temporary care for a pet, confirm that the pet will be returned to the Melita's Dream Animal Rescue, and limit liability. As used in this Agreement: whose address is 21200 Oxnard st #6492 Woodland Hilks, CA 91365 

    Melita's Dream Animal Rescue shall refer to Melita Gvalevich  

    1. Melita's Dream Animal Rescue agrees to place Pet in the temporary care of foster and foster agrees to care for pet as provided herein until it is returned to Melita's Dream Animal Rescue.

    2. Foster agrees to notify Melita's Dream Animal Rescue with changes of address, telephone number and return pet to Melita's Dream Animal Rescue upon request 

    3. As used in this Authorization and Agreement, foster should provide pet with food (given to foster by Melita's Dream Animal Rescue) water and shelter, giving reasonable attention to pet's health, safety and attending to the following special instructions. Otherwise, foster will have no liability to Melita's Dream Animal Rescue except that which might arise irrespective of this Authorization and Agreement. 

    4. Melita's Dream Animal Rescue shall remain the OWNER of Pet, and be entitled to sole custody and possession of Pet at such time as desires. 

    5. Foster agrees that Melita's Dream Animal Rescue shall not be liable if Pet causes foster to sustain injuries or losses, unless caused by a dangerous propensity of Pet (unknown to foster)

    6. Foster agrees that under no circumstances will the aforementioned Pet be sold, transferred, leased, donated or relinquished to any institution, pet store, shelter, individual or other entity 

    7. Foster agrees to give Melita's Dream Animal Rescue at least TWO weeks notice prior of not being able to foster the pet anymore.
